Patient consent
Consent Terms
Template consent information for review. Last updated June 2026.
What you are consenting to
You are allowing your care team to use Episap to prepare for, discuss and document your case in a multidisciplinary team meeting. This may include sharing relevant case information with invited clinicians involved in the meeting.
What may be recorded
The meeting record may include your case summary, clinical question, files or imaging references, discussion notes, recommendations, attendance information, consent status and follow-up actions recorded by the care team.
Who may see it
Access is intended to be limited to authorised clinicians, coordinators and support staff involved in your care or in operating the MDT workflow. Your care team remains responsible for inviting appropriate people.
Your choice
You may consent or decline through the link you received. You may also ask your care team how to withdraw consent or limit future use where clinically and legally possible. Declining means the consent record will show that you did not consent through Episap, and your care team may need to contact you about next steps.
Limits
Episap does not provide medical advice to patients and does not make treatment decisions. Your treating clinicians remain responsible for explaining recommendations, risks, benefits and alternatives.
Questions
If anything is unclear, contact your treating clinician or care team before consenting. You can also ask how your information will be used, corrected or deleted where permitted.
